State Disability Insurance is offered through the California Employment Development Department.

Needed Documentation:
- "Kaiser Permanente Visit Verification Form" with dates of disability obtained thorugh your Kaiser doctor
- Employment Development Department form DE 2501 (patient information page 1, 2 and 4 only)
- Release of information form (can be obtained or completed at Patient Records Department)
To Apply:
- At Kaiser Permanente Santa Clara, claims are processed through Patient Records Dept. 160
Facts:
- Claims takes approximately 4 weeks to process
- Claimant are paid approximately 55% of your salary, if eligible, for up to 52 weeks
- Not paid for first week of disability
- Can claim State Disability Insurance (SDI) for up to one year After job loss
- Retroactive to the day disability began (minus first week)
- Can be used for intermittent or full leave
- Apply no earlier than 9 days before disability began or 49 days after
Disability Insurance Programs:
SDI is a partial wage replacement insurance plan for Californians. The SDI program is state mandated, and funded through employee payroll deductions. SDI provides affordable short-term benefits to eligible workers who suffer loss of wages when they are unable to work due to a non-work-related illness or childbirth.
Private Disability Insurance
Many employers offer private disability insurance programs sponsored by companies such as Matrix, Unum, or VPA. Benefits of these programs vary by employer. Check with you HR representative for details.
Paid Family Leave (PFL)
Paid Family Leave is a state sponsored program that offers benefits to eligible workers suffering a loss of wages due to the need to care for a seriously ill family member or to bond with a new child.
Family Medical Leave (FMLA)
Under FMLA, most employees are entitled to a total of 12 weeks unpaid leave during any 12-month period for the following purposes: birth of a child and the care of such child, placement of a son or daughter with the employee for adoption or foster care, care for a family member who has a serious health condition or for a serious health condition or for a serious health condition of the employee that makes the employee unable to perform the essential functions of his/her position.
