Thank you for considering sharing your experience with us and with those whom you can inspire.
Please email your story in a Word document to SCL-CCC@kp.org.
Due to regulatory reasons, we must review your story before it can be published. If approved by our regulatory advisor, we will then edit your copy and return it to you for approval before publishing. If you accept our edits, we’ll have you sign a form that grants us permission to publish your story. Your signed permission form will be scanned to become part of your medical record. Once the form is received, we will post your story on our site so others can read about and be encouraged by your experience.
